If notification ceases to become necessary at any time during the notification period, you should complete the
application to alter or remove an entry form, and we will remove your entry from the register. We will then write to you to confirm that the entry has been removed. If you have paid by direct debit, you will need to cancel your direct debit instruction yourself.
